Attention All Student Athletes

2007-2008

☺ All St. Mary’s Athletes (4th through 8th grade) must have the following before participating in any extra curricular activity;

1.       Physical Card : signed by a physician, due every two years

2.     Permit Card:  signed by parents and due every year

3.     Code of Conduct:  filled out and signed by parents and students every year

 

All of these forms are provided by and need to be handed in to Mr. McClintock as soon as possible.  If you have questions please call Mr. McClintock at 754-5221 ext.25.
